Band Physical Info

Marching Band Physicals

All students participating in Marching Band are required to have a physical.  Even if you are just thinking about participating in Marching Band, get a physical. These physicals will be valid for both marching band and participation in any other U-E sport during the school year.  If your physical is valid for the first day of the season, August 19th, it is good for the ENTIRE marching band season!

Option 1: School Sponsored Physicals

Sports physicals will take place on May 9th for Middle Schoolers at JFS and May 30th for High School.

  You can go onto the UE website and Athletics to register for physicals.  Here is a direct link. This registration is through Family ID and you will select the date that you would like to have the physical done.  

Thank you as always for being proactive and getting the physicals all set.  No athletes can start or participate on the first day with out a up to date physical.


Option 2: Physicals with Personal Physician 

Physicals can be scheduled with your own physician at your own expense instead of a school sports physicals.

Be sure to bring a copy of the Physical Form with you to your physical.

The district is using FamilyID to collect physical forms and insurance information.   The Marching Band season option is not available until about a month before the season starts, hopefully this will be on July 21st.  At that time,  go onto the UE website and Athletics to register for Marching Band.  Here is a direct link. You will need your insurance information to complete the online forms.  Here is a link to more instructions if you need them.    

Physical forms are due by August 19th.
